How Community Priorities Turn into Real Change
The SCfC Journey Map shows the full journey from community voice to real activities and outcomes. It explains how decisions are made, how funding works, and how learning and improvement happen over time. The journey is circular because community leadership and learning continue year after year.
Community Vision
The journey starts with the community’s vision for children, young people, and families. This vision reflects what the community wants for the future and guides all subsequent decisions.
LCB Priorities
The Local Community Board (LCB) listens to community voices and agrees on the key priorities. These priorities focus on what matters most and where effort and funding should be directed.
Activity Design
Activities are designed to support the agreed community priorities. The LCB guides what types of activities are needed, while the Facilitating Partner (FP) helps shape them into practical, deliverable services.
Community Plan
The Community Plan brings the vision, priorities, and activities together in one document. It clearly shows what will be done, why it matters, and how activities support community priorities.
Funding Eligibility Check (with NIAA)
After the Community Plan is endorsed by the LCB, the FP submits it to the National Indigenous Australians Agency (NIAA). NIAA checks that activities comply with SCfC funding rules before activities can begin.
Activities Begin
Once approved, activities can start in the community. Services are delivered in line with the Community Plan and agreed priorities.
Monitoring and Learning
Information is collected about what is happening and how activities are working. This helps everyone understand what is making a difference and where improvements are needed.
Review and Strengthening
The LCB and FP review activities and outcomes over time. Activities can be improved, changed, stopped, or replaced to better meet community priorities and strengthen impact.
The Journey Continues
Learning from what works feeds back into the community vision and priorities. This keeps SCfC community‑led, responsive, and focused on long‑term change.
